At Monarch, we understand that your time is valuable. To help us ensure that your appointment starts on time, please download and complete the forms below prior to your arrival. If you are unable to do so, please arrive 15 minutes early to complete the necessary forms.

Patient Registration Form Health History

A cosmetic consultation is $100 and will be applied towards any cosmetic procedure cost. We require that your consultation fee be held with a credit card at the time you make your appointment. We politely request 24 hour notice for any cancellations. We reserve the right to charge your credit card for any no-show and/or untimely cancellation.

OUT OF TOWN PATIENTS

Dr. Michaels frequently sees patients that live throughout the country and internationally. We are conveniently located less than thirty minutes from Reagan International Airport and Union Station (Amtrak). There are several local hotels near our office for your convenience.

Out of town patients will typically stay anywhere from a few days to two weeks, depending on your cosmetic surgery.
